Piatak, Mark J. -45 of Portage. Passed away on June 29, 2015 in Johnstown. Born August 6, 1969 in Johnstown. The son of William, Sr. and Irene (Polk) Piatak. Preceded in death by father. Survived by mother: Irene of Portage, Tammy (Thomas) Piatak and children: Jeremy, Jacob and Madison Piatak all of Portage. Also survived by his brother Bill Piatak and wife Sally and their son Jackson all of Halifax, PA, sister: Jennifer Prichard of WA; godfather and uncle Jack Piatak of Portage and numerous aunts, uncles, cousins and friends. Mark was an avid hunter and outdoorsman and a fan of the Pittsburgh Steelers. He enjoyed his family and he loved his children, Jeremy, Jacob and Madison. He was a 1987 graduate of Portage Area High School. Family will receive friends from 4 to 8 PM, Thursday at the Matthew R. Decort Funeral Home & Cremation Services, Inc. of Portage where a service will be held that evening at 8 PM. Fr. Joseph Janiszeski, celebrant. Committal will be private to the family.
